Saxenda is a medication that is used to support weight loss by acting on the brain to decrease appetite and make it easier for patients to reduce their food intake. It also makes patients feel full faster and can be an effective weight loss treatment. This prescription GLP-1 medication is a name brand for liraglutide. It can be used in adult patients considered to be obese (with a BMI ≥30) or adults who are considered to be overweight (with a BMI ≥27) with at least one weight-related comorbidity. It may also be used in some pediatric patients who are aged at least 12 years or older with a body weight of more than 60 kg with weight-related conditions.

This dose is then usually increased to 1.2 mg the next week and then to 1.8 mg the following week. This dose may then be increased to 2.4 mg after an additional week. The final escalation after another week is to a dose of 3.0 mg – the amount of the maximum therapeutic dose. Body weight should be evaluated again after 16 weeks of treatment in adults (this may differ slightly in pediatric evaluation) and Saxenda should be stopped if the patient has not achieved at least a 4% reduction from their starting weight.

Saxenda should be taken once per day under the skin of the upper arm, stomach, or thigh. The pen for this medication may be used by:

  1. Looking at the pen to make sure that the medication inside is clear/without color and that the label on the medication matches what was prescribed to the patient
  2. Attaching a new needle to the device
  3. Checking the flow of the pen (this should be done before the first injection is made from each new pen)
  4. Selecting the correct dose of the pen
  5. Making the injection of the dose in the proper injection site (the site should be rotated for each use) and counting to six
  6. Removing the needle

The pens and the needles used for administering this medication should be properly stored or thrown away after their use. These items should never be shared between patients. This includes the pens – even if a new needle has been attached. Sharing pens/needles increases the risk of developing infections.

This medication comes with a boxed warning that has been issued by the FDA for the medication’s potential to cause C-cell tumors. This is an effect that was observed in studies done with rodents. The human risk is still unknown. The medication should not be taken by any patient who has previously been diagnosed with Medullary Thyroid Carcinoma for this reason. The same goes for any patients who have previously been diagnosed with Multiple Endocrine Neoplasia type 2 or who has had a family history with either of these issues.

Saxenda should also not be used by any patients who have a hypersensitivity to liraglutide or any of the other ingredients used in the making of the drug. Patients taking this medication should discontinue Saxenda right away if pancreatitis is suspected or if this condition has developed. Acute gallbladder disease may also occur in some patients taking this medication. Tests should be run by a doctor to check the patient’s gallbladder if they show symptoms that suggest cholelithiasis or cholecystitis.

Patients have a greater risk of developing hypoglycemia while taking this medication if they are also taking an insulin secretagogue or insulin medication at the same time. Patients using these medications together may require dose adjustments before starting Saxenda. However, the risk of developing hypoglycemia is low when Saxenda is used as a monotherapy. This medication can also lead to an increase in heart rate. Patients using this medication should have their heart rate monitored on a routine basis.

Postmarketing surveillance has shown some instances of patients experiencing renal impairment. This is usually a secondary effect that develops due to dehydration. Patients with kidney impairment should exercise additional caution when initiating or increasing doses of Saxenda. Anyone taking this medication with a history of depression or suicidal thoughts should be closely monitored.

This medication is also contraindicated in pregnant women and has the potential to lead to fetal harm. This medication is also not recommended for use in those who are breastfeeding since there is limited data to show the effects this would have on the baby. Doctors and healthcare providers should be made aware of any conditions the patient has or medications/supplements that the patient is taking before they start Saxenda.

Not every patient who is taking Saxenda will end up experiencing side effects. However, patients should still be informed about the most commonly reported ones. These include those such as:

  • Diarrhea or nausea
  • Enzyme level changes in the blood
  • Vomiting or constipation

  • Fatigue
  • Headache
  • Hypoglycemia (mainly a concern when used with insulin or insulin secretagogues such as sulfonylureas)
