Children aged 10 and older require particularly close monitoring for systemic side effects during the mandatory four-week dose-escalation phases. A doctor may increase this dose to 1.5 mg once per week for at least four more weeks if the patient’s blood sugar levels remain high. A doctor may also determine that a further dose increase to 3 mg per week may be needed after those four weeks if blood sugar levels need further management. The maximum dose a patient may be prescribed of this medication is 4.5 mg if blood sugar levels remain high after at least four weeks after being on the 3 mg dose. Each of these pens comes prefilled with 0.5 mL of the medication solution.

Patients can take their weekly doses of their medication from their Trulicity pens by:

  1. Removing the pen from storage and checking the label/expiration date
  2. Checking the pen to make sure it is not damaged and the liquid inside of it is clear/free of particles
  3. Thoroughly washing their hands
  4. Locating the injection site (this site should be rotated for each injection that is made and must be on the abdomen, thigh, or upper arm)
  5. Uncapping the pen
  6. Placing the clear base flat against their skin
  7. Turning the lock ring to unlock
  8. Making the injection by pressing the green button and listening for a click
  9. Continuing to hold down the button until a second click is heard (this often takes about 5-10 seconds)
  10. Gently lifting the pen away to remove it from the skin
  11. Checking to make sure the gray plunger in the pen is able to be seen (indicating that the dose is complete)
  12. Disposing of the used pen in a proper sharps container

Each pen is single-use and pens should never be shared between patients. They should also not be used if they become frozen or if the solution does not appear clear. Patients may choose whether or not they take their doses with food.

Trulicity has a boxed warning for being possibly linked to thyroid C-cell tumors. This may include cancer. This possible link was found during rodent testing and how these effects translate to use in humans is still uncertain. This is why patients are told not to take this medication if they themselves or any member of their family have had a history of medullary thyroid carcinoma. The same goes for those with personal or family histories of Multiple Endocrine Neoplasia syndrome type 2. This medication is considered to be contraindicated in both of these populations.

Those who are taking this medication and believe they are experiencing any signs of thyroid tumors – which may include signs such as a lump or swelling in the area of their neck – should seek immediate medical attention. This also includes if the patient starts experiencing side effects such as hoarseness in their voice or trouble swallowing. This medication is also contraindicated in anyone who has experienced prior hypersensitivity reactions to any of the contents of Trulicity.

Clinical trials have shown that pancreatitis may also occur in patients taking this medication in some cases. This medication should not be used if this condition is suspected and it should not be continued if this condition is confirmed. The development of hypoglycemia is also possible when taking this medication if it is being combined with the use of any insulin secretagogue or insulin medications. A reduction in the dose of concomitant insulin or insulin secretagogues is often required to minimize this risk.

Kidney function should be monitored for patients with renal impairment. Monitoring is primarily required when patients experience severe gastrointestinal adverse reactions that could lead to dehydration and acute kidney injury. The severe gastrointestinal effects that may possibly occur from this medication also mean it is not recommended for use in patients with severe gastrointestinal disease – as the use of this medication has not been studied in this group of patients. Trulicity should be avoided in patients with pre-existing severe gastroparesis because it has not been adequately studied in that population. Cardiovascular outcome trials also have found that diabetic retinopathy may occur for some patients taking this medication. However, temporary worsening of retinopathy is actually associated with the rapid improvement of blood glucose levels rather than the medication itself. Patients with diabetic retinopathy in the past should be closely monitored.

Common Side Effects

The side effects of this medication that are most frequently reported by patients are those such as:

  • Fatigue
  • Indigestion
  • Nausea
  • Decreased appetite
  • Abdominal pain
  • Vomiting
  • Diarrhea

These side effects are most frequently experienced when the patient starts their medication or around the time of dose increases. Most of these side effects should resolve on their own as the patient’s body gets used to the medication. Patients whose side effects do not go away or whose side effects become severe should seek medical attention.

Serious side effects may also occur for some patients. However, these effects are less frequently reported. These include side effects such as:

  • Pancreatitis (patients must immediately stop the drug and seek help if they experience persistent, severe abdominal pain that radiates to the back)
  • Gallbladder issues
  • Hypoglycemia (more common in those combining the medication with a sulfonylurea or insulin medication)
  • Kidney problems (usually caused by dehydration or a loss of fluids while taking this medication)
  • Severe stomach issues (that go beyond gastrointestinal intolerance during initiation or dose escalation)
  • Hypersensitivity reactions
  • Vision changes
  • Allergic reactions
